Canine Heritage™ Breed Test Photo Submission

This form will allow you to submit a digital photo of your dog for the Canine Heritage™ Breed Test. If a photo is uploaded for your test, it will be printed on your Certificate of Canine Breed Analysis at no extra charge.

Important - Please Read!

  • A photo can ONLY be submitted once you have purchased and received your Canine Heritage™ Breed Test Kit in the mail.
  • Have your sample submission form available and identify the unique Test ID number at the top right hand corner of your form.
  • Only JPEG (file extension .jpg) photos are accepted
  • Digital photos must not exceed 2MB (megabytes) in size or have a resolution of 2400x1800 or less
  • If your Test ID is entered incorrectly, your photo may not be printed on your results certificate
  • If you are submitting photos for more than one sample, please submit a separate form upload for each test
